At Perfect Balance Physical Therapy, we strive to provide the best care and ensure that our

patients receive the attention they need. To help us achieve this, we have established the

following cancellation policy:

Cancellation Fee: There will be a $75 charge for visits cancelled with less than 48 hours'

notice. This fee is necessary as we are unable to offer your reserved time to another

patient without sufficient warning.

Rescheduling: We understand that unforeseen circumstances can arise. We are happy to reschedule our appointment to a more convenient time. Please contact us as soon as possible to arrange a new appointment. 

We appreciate your understanding and cooperation in helping us maintain a high standard of care for all our patients.

When you call to have us schedule your first appointment, please have your prescription (if applicable) and insurance card handy. This will allow us to confirm your insurance benefits and give you a coverage estimate.

To bring/keep in mind on your first visit:

1. Please wear comfortable and loose clothing that allow access to skin on body part being evaluated. 

2. Bring anything you would like your therapist to know in the form of  medical or imaging reports or a MD prescription, if you have one. Please note: PTs DO NOT NEED A RX from an MD, in CA. 

3. You will be emailed a Medical Health Questionnaire and questions about your function and pain. This should be completed prior to your Initial Evaluation to allow for complete Electronic Medical Records. Please answer all questions on the survey, even if it isn’t exactly clear to you. We can help address questions when you arrive in office. 

4. The completion is required for proper documentation regulations and allows for higher reimbursement from the insurance company. Please look in your Spam folder if you do not see it.

We accept full payment prior to time of service. We accept health, savings accounts, flexible, savings accounts, checks, cash, credit cards with a fee. We like Square Merchant Services and Gene Services.Understanding your out-of-network benefits for physical therapy and how to get reimbursed by your insurance company can help alleviate some of the financial burden associated with seeking treatment outside your insurance network. You need to Check Your Insurance Coverage and understand what you can expect t be reimbursed. You need to  be prepared obtain details about your specific insurance plan to understand your out-of-network benefits, including any deductibles, co-pays, or coinsurance you may be responsible for. 

You DO NOT need permission from a doctor, or anyone else to come to physical therapy! California is a Direct access state, and does not require an MD prescription. No Rx is required to attend PT, but we’re always interested in seeing one if you have it. 

A referral serves a valuable role as communication between the physician and your PT. This is helpful information, but not necessary, as we have excellent evaluation skills and capacities. So text, email or bring it with you if you do have a prescription or referral from your doctor. We will happily connect with your MD and can provide them reports about your ongoing PT recovery journey. We can fax an initial evaluation and progress reports to any of your doctors, upon request. For an optimal recovery it's important to have ongoing communication between all of your caregivers. A health & wellness team, working together, can provide you an integrated medicine approach which science has shown to result in complete healing and recovery.
